    **Form #1 must be filled out in full before each season a student is playing a sport. It must be turned in to the school nurse.
     

    **Form #2 is used for a physical being done at the school. It must be filled out and signed by a parent.

     2018 FALL MAKE UP SPORTS PHYSICALS  

     
     
     
    WHO:        7th - 12th Graders Planning on Playing Fall Sports
     
    DATE:       June 5th and 6th at ETMS and June 7th & 8th at NWHS
     
     
    TIME:        9am
     
    WHERE:  Niagara Wheatfield Gym 
     
       
     

     

          All District sports physicals will be performed by the staff of physicians and nurse practitioners from University Sports Medicine of Niagara.

      
          If your child plans on participating on a Modified, JV or Varsity athletic team, in the upcoming 2018-19 school year, they must obtain a physical during the above time.
     
          If your child has a history of asthma, severe allergy/epi-pen, or uses an inhaler, a Health Care Plan and Medication Form must be completed by your family physician and returned to the Health Office. These forms can be picked up in the Health Office, or downloaded from
    the school website. Should the condition no longer be a problem, please send in medical documentation so that your child’s health file can be updated.
     
          Every student must have a completed and parent signed copy of the attached “Pre-participation
    Physical Evaluation” form. 
     

          Students who wear glasses or contacts must have one of them with them at the time of the physical.

     
     
    SPORTS PHYSICALS: 2018-2019 SCHOOL YEAR  REQUIREMENTS FOR SPORTS PHYSICALS
    • "Pre-Participation Physical Evaluation" form must be completed on both sides and signed by parent(s)/guardian(s).
    • "Health Care Plan" completed and signed by parent(s)/guardian(s) and/or physician
       (if applicable).
    • Glasses or contacts for vision test, if required, for distance vision.
    • Medical Clearance - If your child was not in Physical Education classes or has been on limitations, a note from the treating physician must be submitted stating clearance to return to gym/sports.

    FAILURE TO SUBMIT ALL REQUIRED PAPERWORK WILL PREVENT ATHLETES FROM STARTING PRACTICE IN A TIMELY MANNER.
     
     
    You must obtain a sports physical at the above time with our sports doctors.
    If you do not have a physical by our sports doctor you WILL NOT be able to start tryouts.

     
     
     
     
     
     
    *****These Sports Physicals meet the requirements for the mandated physicals that students need for entering 7th and 10th grades and do not have to be repeated.*****
